Providing health care to the patients and managing health care facilities, a care manager works in a number of capacities. Indeed, these care managers serve as the personal advocates for the health and well-being of clients. They are here to ensure that all aspects of care are well coordinated and medical transitions go as smoothly as possible. Apart from creating a plan of care, the care managers are also responsible for providing other invaluable forms of support. These include transitional support, continuity of care, family and client education, and coordination of client resources.

Emerging as a primary means of managing the health of a defined population, care management is aimed at decreasing the health risks and health care costs. After all, an effective care management can improve the disease control and self-management, thus reducing distress, preventing admissions and improving the coordination of care for more complex patients.

In case of a medical distress, it is quite challenging to keep a clear path of communication between physicians, family members and caregivers. This is when a care manager comes into play. He will attend the appointments of the doctor and will a push for further testing when they see a gap in care. Besides, he will speak up for the wishes of the client and will ensure that all the entities are well informed and communicated clearly. Thus, these care managers play a significant role in the client and family education. He is the one who will help you make informed decisions about the healthcare. After all, they connect the patients to the tools, thus making their life much easier and fulfilling.

Serving as the program director and care manager at Brockton Area Multi-Services, Inc., Galen Stone worked on developing and holding multiple roles in pilot program for the state. Besides, he also served as the program director and care manager of CFFC program in Brockton. This program was created as a response to the lawsuits initiated by parents due to their children receiving inadequate services through state run programs.

Here, Galen was solely responsible for coordination of wraparound services for more than 50 at-risk families within Brockton, facilitating meetings and services on all fronts on behalf of the children and their families. He conducted home-based therapeutic services while working with youth and their families.
Galen was also responsible for supervising all clinicians, psychiatric nurse, and consulting psychiatrist regarding children and family needs and welfare.
