Thankfully, you are blessed with a great and blissful life. Even a thought of the chronically ill patients will shatter you from inside. The patients with chronic illness feel great difficulty in participating in everyday activities and it gets worst with time. Frankly speaking, they are unable to fulfill at least two activities of daily living that includes bathing, eating and dressing for at least 90 days. They always demand substantial supervision and assistance in order to protect themselves against various health and safety threats. For such chronically ill patients, proactive care management is required to reduce the risk of complications. In addition, it helps prevent the costly hospital admissions.

Otherwise heading towards deterioration and readmission, a well-organized care management by a nurse or any other health professional can reduce this risk among the patients. Indeed, an effective care management will improve the disease control and self-management, thus improving the coordination of care for more complex patients.

Being a care manager demands many responsibilities. He acts as a core member of care team that involves the primary care provider of the patient, a consulting psychiatrist and other mental health providers. They serve as the personal advocate for the health and well-being of the patients. He is solely responsible to make sure that all aspects of care are coordinated and the medical transitions go as smooth as possible. Besides, care managers also provide other invaluable forms of support. All these work towards the high quality of care, better outcomes for the client and prevention of readmission's to the hospitals. Care management can be an invaluable resource to the clients and their families. The care managers come up with the proven strategies to identify the potentially appropriate patients.

Focused at providing the best care to the patients, Marion based Galen Stone served as the Program Director and Care Manager at Coordinated Family Focused Care at Brockton Area Multi-Services Inc. Here, he worked for CFFC program, which was created by Massachusetts Behavioral Health Partnerships. It was created in response to the lawsuits initiated by the parents as the children were receiving inadequate services through State run programs.

Galen Stone was solely responsible for coordination and implementation of wraparound services for more than 50 at risk families within Brockton, thus facilitating meetings and services on all fronts. He also supervised all clinicians, MSW interns, psychiatric nurse, and consulting psychiatrist regarding children and family needs and welfare.
