Center for Hospice Care Works with The Hospice Foundation to Aid Patients and their Families

Center for Hospice Care provides palliative and hospice care for patients with life-limiting illnesses. It receives support and donations from the Hospice Foundation.

 
[b][South Bend, 09/22/2017][/b] - Patients with life-limiting illnesses in the advanced stages are not the only ones who need support and care. Their families need them, too. This is why Center for Hospice Care has partnered with the Hospice Foundation, a non-profit organization. With the foundations support, the hospice can carry out its mission to improve the quality of life of patients and their families through hospice and palliative care.

Located in South Bend, Indiana, the hospice works with hospitals, social workers, and the clergy from all over the region. It accepts referrals and provides information and support to the individuals and families in need.

[b]Outpatient and Inpatient Palliative Care[/b]

Center for Hospice Care has two inpatient units in South Bend and Elkhart. Here, the center can provide short-term care for patients who, after assessment, will benefit most from hospice care in a home-like setting.

The center is also available to patients who are living in their own or assisted living and nursing homes. At least one staff member is available 24/7 via telephone (no fees required). The center’s staff and volunteers are always ready to offer assistance in cases of emergencies. They also provide emotional and spiritual support for patients and their families.
