The stem is attached to a slot on the top of the ball, allowing it to rotate a quarter turn (90 degrees). The shaft allows for some lateral movement of the ball caused by the upstream pressure acting on it. This minor lateral movement, in fact, creates a load on the ball, pressing it against the downstream seat, improving the valve's leak tightness. This type of valve can shut off in both directions. When the upstream pressure is high, the floating valve is very difficult to operate.
