The Support Services Program of Hospice of the Calumet Area Assists People Adjusting from Illness

The Hospice of the Calumet Area has a support services program called Transitions™, which aims to assist patients and their families who are adjusting to a life-limiting disease.

 
[Munster, 05/15/2017] — The support services program of the Hospice of the Calumet Area, called Transitions™, helps patients and their families who are adjusting to a terminal disease. The program aims to unite caregivers and clients during a period characterized by confusion and stress.

Services Provided by Transitions Staff and Volunteer

The Transitions™ support team works with patients and families during this difficult time. The team helps individuals cope with the difficulties of the illness and meet their particular needs by offering companionship, assisting with daily tasks, and locating referrals to community resources.

A Transitions™ staff can specifically provide assistance in coping with the struggles of a life-limiting illness, deciding on care options, and searching for community resources to assist the dying individual. On the other hand, a Transitions™ volunteer can specifically offer assistance with errands and meal preparation, rest breaks for caregivers, and companionship visits.

Patients and families need not shell out money to take advantage of these services. The Hospice of the Calumet Area can provide Transitions™ at no cost thanks to community grants and donations.

Helping Families Grieve through the Hospice’s Program

When a patient passes away, the hospice is there to help families through this tough time. The hospice’s comprehensive bereavement program provides support for families in search of the hope and strength they need to recover. Some of the services offered by the bereavement program include grief support groups, educational resources on bereavement and grief, monthly mailings, and short-term counseling and support services.
