The DOCSUN Clinical Computation Lab has issued this guideline as part of an emergency temporary standard (herein known as ETS) to protect healthcare frontliners and service/support workers in the labor sector from occupational exposure to COVID-19.

 
Purpose

The DOCSUN Clinical Computation Lab has issued this guideline as part of an emergency temporary standard (herein known as ETS) to protect healthcare frontliners and service/support workers in the labor sector from occupational exposure to COVID-19 in settings where people with COVID-19 are reasonably expected to be present, mainly via the DOCSUN Doorway DC1000 Self Diagnostic AI Tool.
During the period of such emergency standards, employers must develop and implement a COVID-19 plan to identify and control COVID-19 hazards in the workplace. Apart from the use of DOCSUN’s self-diagnostic AI tool, healthcare covered employers must also implement other requirements to reduce transmission of COVID-19 in their workplaces, including but not limited to:
• Patient screening and management;
• Standard and Transmission-Based Precautions;
• Personal protective equipment (PPE), including face masks or respirators; controls for aerosol-generating procedures;
• Physical distancing of at least six feet, when feasible;
• Physical barriers;
• Cleaning and disinfection;
• Ventilation;
• Health screening and medical management;
• COVID-19 Protective Measures Training, recordkeeping and reporting.

This guideline also encourages vaccination by requiring employers to provide reasonable time and paid leave for employee vaccinations and any side effects. It also encourages use of respirators, where respirators are used in lieu of required facemasks, by including a mini respiratory protection program that applies to such use. Finally, the standard exempts from coverage certain workplaces where all employees are fully vaccinated and individuals with possible COVID-19 are prohibited from entry; and it exempts from some of the requirements of the standard fully vaccinated employees in well-defined areas where there is no reasonable expectation that individuals with COVID-19 will be present.

Scope and Application

This guideline applies to employers in settings where any employee provides
healthcare services or healthcare support services. This includes: employees in hospitals, nursing homes and assisted living facilities; emergency responders; home healthcare workers; and employees in ambulatory care facilities. These settings are collectively referred to as “healthcare workers” in this document.
The focus of DOCSUN’s Labor Workforce Guideline and ETS is on protecting healthcare workers in settings where suspected or confirmed COVID-19 patients are treated, specifically in settings that provide treatment to patients with suspected or confirmed COVID-19 face a particularly elevated risk of being infected with SARS- CoV-2 because the delivery of such care requires repeated instances of close contact with infected patients and healthcare workers will be of greater risk of exposure to new SARS-CoV-2 variants because they will be among the first to be exposed to people carrying the variants as they emerge and those infected seek medical care. However, we do not distinguish between healthcare services provided outdoors from those same services provided indoors as everyone must be covered equally. For example, the risks to an emergency medical technician who provides mouth-to-mouth resuscitation to a patient are the same whether the care is provided outdoors or indoors.

Prerequisites

DOCSUN’s Self Diagnostic AI tool and scanning system are originally intended to be used for the prediction of the physiological state of the user and identifying if the user is infected by Covid-19. A Diagnosis Summary is generated based on the prediction of the Physiological state of the user, the summary contains information of symptoms of infection or normal state.

Principles

The DOCSUN Doorway DC1000 Self-Diagnostic Tool essentially identifies the infection on the lung X-ray image through the area of coverage of the “white matter” on the x-ray image of the lung, this understanding helped us to develop an AI model trained to classify between the images that were labelled as the infected and the normal cases. The question of how the AI model was able to identify the presence of the white matter on the image motivated us to utilise the textual report data, to estimate the vital signs of the patient from the reports, this statistical research proved the bracket of consideration of the physiological parameters in cases of infection. Based on this bracket we fine-tuned our approach to the problem which led to the development of the Self-Diagnostic Covid19 Software. The software helps identify the physiological state of the human body through tracking of the core vital signs of the human body. To reduce the intrusiveness in the detection of the diseases, we have adopted the principle of Imaging Photoplethysmography (iPPG).


Procedure

DOCSUN’s COVID-19 Diagnostics system is a type of Imaging Ballistocardiograph (iBCG) measurement software and hardware for end-to-end video-based monitoring of pulse-rate, respiration-rate, spo2-level, temperature, and red eye, which has been fine-tuned with over 20,000 records of symptoms to correspond for respiratory diseases stored in our medical data bank. DOCSUN’s AI tool software uses custom algorithms along with SOTA AI systems to deliver accurate results. By using real time video monitoring, we provide non-intrusive therapy to the patient. The Diagnostic Covid19 makes use of custom AI algorithms in ensemble to analyse the Imaging Photoplethysmography, this provides the analysis of blood volume pulse (BVP). As stated above and to reiterate, the basic principle is based on clear and subtle changes in the light reflected from the skin, to reduce the signal to noise ratio involved, we consider the motion factor and apply denoising to the extracted signals, allowing for accurate prediction of the physiological parameters.
The Diagnostic Covid19 Software has been deployed on our official website and will be made available to the users as Mobile Applications supported across Android and IOS Devices so any laborer with a phone can use it for free. Users who are in quarantine or need a confirmation on their state of illness can upload or record a video to the application and get the diagnosis results which can be shared to their medical consultant for further diagnosis.

Provisions

DOCSUN’s ETS generally includes provisions that are based on and in accordance with applicable CDC and other well-established guidelines for good infection control practices relevant to the exposures encountered by employees during their job tasks. For example, the ETS requires the employer to develop and implement policies and procedures to adhere to Standard and Transmission- Based Precautions. As discussed in detail in the above sections, these requirements are consistent with well-established CDC and other guidelines that are routinely followed by employers subject to the ETS. That the ETS is based on CDC and other guidelines or practices that are well established and have been routinely followed by many employers both before and during the pandemic is compelling evidence supporting DOCSUN’s finding of economic feasibility with the use of its protective systems.

Standard and Transmission-Based Precautions

Standard and Transmission-Based Precautions are well-accepted as important to controlling disease transmission.
It should be noted that during times of significant transmission, such as during this pandemic, additional protections are needed to supplement the basic level of recommended precautions and practices in these guidelines. For instance, wearing at least a facemask regardless of interaction with known or suspected infectious patients is needed during the pandemic.
Standard Precautions refers to infection prevention practices, implemented in healthcare settings, where the presence of an infectious agent is assumed (i.e., without the suspicion or confirmation of exposure). The use of Standard Precautions thus relies on the assumption that all patients, patient samples, potentially contaminated materials (e.g., patient laundry, medical waste), and human remains in healthcare settings are potentially infected or colonized with an infectious agent(s). For example, Standard Precautions would include appropriate hand hygiene and use of personal protective equipment as well as practices to ensure respiratory hygiene, sharps safety, safe injection practices, and sterilization and disinfection of equipment and surfaces.
