We call rash the appearance of one or more red spots on the skin. The rash may be asymptomatic, but most often it is accompanied by other symptoms, such as itching, fever, or local pain.

 
As there are dozens of diseases that can cause red spots on the skin, from allergies to infectious pictures, the presence of these other associated symptoms help us to establish the most likely diagnoses.

1 - URTICARIA
Urticaria is one of the most common causes of itchy rash. About 20% of the population has at least one episode of the disease in their lifetime.
Allergic urticarias are the most common and usually appear soon after contact with the causative agent. The time varies from a minutes to few hours. The main causes are:
• Antibiotics.
• Aspirin.
• Anti-inflammatory.
• Insect bites, especially bees and wasps
• Contact with latex.
• Foods such as eggs, milk, seafood, soy, dyes (even natural ones), nuts and peanuts.
• Contact with substances, including plants, pollen, animal saliva, paint, resins, perfume, detergent ...


Although it is often treated as an allergic process, the truth is that urticaria can also be triggered by non-allergic reactions. Viral or bacterial infections, parasites, exposure to heat or cold and physical exercise are just a few examples of non-allergic triggers of urticaria.
Urticaria skin lesions are usually multiple, reddish and itchy. The stains usually have relief and can have varied sizes, but are, in general, always round. In some cases, the red spots may agglutinate and form large plaques. When pressed with the finger, the urticaria lesion tends to evanesce in an ephemeral fashion, returning to the red color as soon as we remove the finger.
Acute urticaria condition may lasts for some days, usually upto five days, but there are cases that may take a little longer to go away. Throughout the day new lesions may appear, and the older ones may disappear, creating a transient and migratory rash pattern. Each lesion usually disappears within 24 to 36 hours.

2 - PITYRIASIS ROSEA
Pityriasis rosea is another extremely common cause of red spots on the skin with intense itching. We do not know exactly why the disease arises, but studies suggest that it is caused by some of the viruses in the human herpesvirus family, such as human herpesvirus 6 (HHV-6) and human herpesvirus 7 (HHV-7) which cause in children a completely different disease, called sudden exanthema or infantile rosea.
Although it is a skin eruption of probable viral origin, pityriasis rosea is not contagious.
In most cases, the rash starts with a single rounded, pinkish-colored lesion with slight flaking and usually the size of a coin. A few days later, new lesions, similar to the first one, but of smaller size, begin to appear by the trunk and around the groin. These red spots can be very pruritic and take 1-3 months to disappear. In this period, new lesions may arise along the body.
As the days go by, the lesions become ring-shaped, with a clearer center and reddish edges and with a scaling.

3- ATOPIC DERMATITIS - ATOPIC ECZEMA
Atopic dermatitis, also called atopic eczema, is a rash of skin that is usually pruritic, especially at night. It is a very common eruption in infants and children, but it can also affect adults. In large number of cases, atopic dermatitis, it affect in initial 5 years of life.
It is a chronic skin ailment, where there is improvement along with relapsing phases of rash worsening.
The causes of atopic dermatitis are unknown. Although the name suggests an allergic process, we now know that atopic dermatitis is not an allergy, but rather a problem that arises from genetic defects of some skin proteins.

Atopic dermatitis usually arises in areas of folds, such as the folds of the knees, the neck, and the folds of the elbows. The signs and symptoms of atopic dermatitis can vary greatly from person to person. The most common are:
- Itching, which gets worse at night.
- Reddish / brown spots or plaques, especially on the hands, feet, ankles, wrists, neck, upper thorax, eyelids, elbows and knees. In children, the face and scalp also tend to be affected.
- Dry, scaly or cracked skin.

4- MOSQUITO BITE
Mosquito bites are common causes of red spots on itchy skin. During the bite, the mosquitoes inject their saliva, which has anticoagulant action It is the saliva of the mosquito that usually causes the allergic reactions in people.
The symptoms of the bite usually appear within 20 minutes and can take up to 2 days to disappear. The more reactive a person is with the saliva of the mosquito's, the more intense the allergic reaction is. Initially, the lesion may be elevated, with a lighter center and reddish edges. As the days go by, the lesions become flatter and homogeneously red.

5- CONTACT DERMATITIS
Contact dermatitis is a skin irritation caused by direct contact with some irritating substance. There are two forms of contact dermatitis. The most common is that caused by skin contact with naturally irritating substances, ie substances that cause irritation in practically everyone. The 2nd type is the skin contact with the substances to which the person is allergic. They are generally substances that do not provoke reaction in the skin of the majority of people, only in who is allergic, like for example, people who have allergy to gloves of latex.
Several substances and objects can cause contact dermatitis, among them: jewelry, poison ivy, latex, perfumes, rubber, leather, soaps, etc.
The main sign of contact dermatitis is the appearance of a reddish rash, which in allergic form causes itching, and in the irritative form causes burning. Small blisters and skin cracks may also arise. Contact dermatitis lesions tend to be restricted to areas of skin that have come in contact with the harmful substance.
In most cases, this dermatitis disappears after a few days if the patient no longer has contact with the substance that triggered the injury.

6- DERMATOPHYTOSIS (SKIN RINGWORM)
The superficial mycoses of the skin are called dermatophytosis or tinea. Dermatophyte fungi, that is, that cause dermatophytoses, are those of the genera Trichophyton , Microsporum or Epidermophyton . Dermatophytosis is a mycosis other than candidiasis, which usually causes intertrigo (see next topic)

Dermatophytoses can affect various areas of the body, such as scalp, feet, beard, nails or trunk and limbs. Skin mycoses are contagious and can be transmitted by contact with bedding, towels, or regular clothing.
The main symptoms of skin ringworm are itching and local redness. Tinea usually begins with a reddish, oval-shaped plaque or patch, the center of which tends to brighten as the days go by. If left untreated, the lesions usually expand in circles. Tinea cruris, which is the mycosis that affects the groin, may also present as a large red plaque on the inside of both thighs.

7- CANDIDIASIS
Candidiasis intertrigo is a mycosis of the skin caused by the fungus Candida, which affects the intertriginous areas, that is, areas where there is contact of two skins, such as the groin, armpit, scrotum, lower breasts or region between the fingers.
These areas of folds are especially susceptible to the appearance of intertrigo, because they are humid and hot regions, which favors the proliferation of germs, mainly fungi.
Candidiasis of the skin manifests itself by very reddish plaques on the skin, with slight flaking and small red spots around it. These lesions can scratch or burn and are located in areas of skin folds.
In babies, skin candidiasis is common in the areas covered by the diaper.

8- SCABIES
Scabies, commonly known as scabies, is a skin disease caused by the mite Sarcoptes scabiei . Scabies is a contagious infection that can spread quickly through close physical contact, such as occurs between people who live in the same household or with children in daycare centers.
The classic symptom of scabies is a diffuse body itch, which tends to be more intense at night. The typical lesions of scabies are small dots or reddish and embossed marbles. In some cases, the lesions may be so small that they can be camouflaged by the scratches caused by the intense itching.
The sites most affected by mange are the hands, wrists, elbows, armpits, nipples, areas around the navel, genitalia, knees, buttocks, thighs and feet. The back is usually spared and the head, palms and soles are usually only inflicted on children.
